Season 4, Episode 1: A Change Is Gonna Come

Meredith, Izzie, Cristina, and Alex have become residents and are put in charge of the new interns at Seattle Grace. Meredith find out that her half sister Lexie is among the new interns. Callie struggles with her new role as chief resident, while Bailey is upset because the job was not given to her. Bailey's bad attitude causes problems between her and Callie.

George tells Callie that their marriage is over. Derek and Meredith decide to just have sex, instead of being in a relationship. After a gas main break blew in their building, five patients are brought into the pit. Mama Burke shows up to the hospital. Callie will not allow a resident to oversee the clinic.

A woman named Marcy bring in her son, due to suspicious behavior changes. Unless Mark and Richard can perform a new surgery, a patient named Connie may lose her ability to talk. George deals with his guilt about having sex with Izzie. Really Old Guy, a comatose patient from previous seasons, wakes up.

Callie announces to everyone that Izzie is sleeping with her husband. Richard needs a new place to live, as Adele files for divorce. Bailey has a patient who claims that his foot that doesn't belong to him. Norman is in Alex's group of interns today and asks if he can go home because he is not feeling well.

Bailey's marriage continues to suffer as she spends more time trying to save lives, and less at home with her family. Her husband has trouble accepting her devotion to her job. Cristina tries to improve her relationship with Hahn so she can work with her.

Bailey is distraught when her young son arrives at the ER needing medical attention. Derek pushes Meredith do commit more to him, and Meredith struggles with the pressure. Callie tries to cope with her and George no longer being married. George is surprised by a visitor he had no idea was coming.

Rose tells Lexie that she loves Derek, as Meredith listens in. Webber scolds Bailey, for keeping her residents at the hospital for two weeks. Meredith wins a surgical contest, which means she gets first dibs on all surgeries for the nest three months. George decides he wants to stay in the apartment with Lexie.

Addison Montgomery comes back to consult on a difficult pregnancy. She's amazed to see how much has changed at Seattle Grace, with Derek and Meredith broken up and Callie divorced. Babies are the spirit of the hour, as Rebecca also returns to tell Alex that she is with child. However, this turns out to be false. Izzie also explores her relationship with the interns in the free clinic.

The nurses at the hospital refuse to work with Mark, due to his numerous trysts, which causes the hospital to instate a "date and tell" policy. Cristina tries to work through her feelings for Burke, while Meredith continues her therapy. Meredith and Derek's newest clinical trial patient turns out to be a gay veteran soldier.

Cristina is still in a slump and her behavior towards others begins to get more and more volatile. Izzie finally confronts Ava/Rebecca. A patient with a Cinderella story affects Meredith, who has been quite angry with her therapist. Bailey works with her child, who has been having issues with hitting other children at day care. Derek and the Chief both have trouble with the women in their life.

While a surgical team races the clock to save a boy who has been encased in cement, Derek and Meredith are given one last chance to work on their clinical trial. Alex's judgment is questionable as the painful situation with Ava brings up past memories that are better left alone.
